Start with the affected person, then trace the harm pathway. “Bias” is too broad on its own—name what happens, to whom, how serious it is and what would reduce it.

Reduce

Add fairness testing, meaningful human review, transparency, redress and monitoring.

Use supplier obligations and assurance, while keeping organisational accountability.

Accept

Document and approve only an understood residual risk within risk appetite.

Monitor

Track outcomes, overrides, complaints and drift with thresholds and named owners.
